I thought Kin On was a nursing home.
Kin On Community Care Network is a non-profit, community-based organization that provides an array of services to the Asian community in King County. Funded by the City of Seattle, United Way of King County, SHARE Awards grant and individual donations, the Kin On Community Care Network exists to meet the health needs of elders and disabled adults who choose to live at home rather than at a nursing facility.
Staffed with bi-lingual and bi-cultural professionals, Kin On Community Care Network offers:

Who is eligible for enrollment in the Kin On Community Care Network?

Persons 18 and above with some kind of disability

How much does it cost to enroll in the Care Network?
Membership in the network is free. The network is made available through the support of individual donations, community efforts, and grants to Kin On. However, donations from the enrollees are welcome.

Who pays for the services provided through the Network?
Services may be paid for through third party payment such as Medicare, Medicaid, and private insurance. Because there are distinctive eligibility guidelines for such services and benefits, Kin On cannot guarantee that enrollees will receive services or benefits. Private pay options are also available.

What is the role of my physician?
Your primary care physician will continue to offer diagnosis and treatment of illness, recommend referrals to specialty care, and conduct wellness appointments. As an enrollee of the Community Care Network, your Care Manager will coordinate with your physician regarding longer-term care, enabling you to continue living independently in your home.

What is the Care Manager's role?
Your Care Manager will have training in social work and/or nursing. He/she will work with your physician and a team of social and health care providers both in and out of the Kin On network to coordinate your care. Your Care Manager will help you access the services you need, and answer any questions that you have.

What benefits do I gain by enrolling in the Community Care Network of Kin On?
All Community Care Network of Kin On enrollees are entitled to the following benefits:

A Care Manager-someone who will work with you to create your own, personal health and safety plan. As your needs change, the Care Manager will make sure you receive the specific care and services you require.

One, free in-home safety inspection.

Free subscriptions to the
Kin On Health News, a quarterly
newsletters providing bilingual community, health, nutrition, and safety information.

Appropriate language assistance such as Cantonese, Mandarin and Toishanese.

Appropriate billing practices. While enrollment is free, Community Care Network services may be paid through third parties such as Medicaid, Medicare, or private insurance. Private payment plans are also available.

Your Care Manager will assist you in determining which programs you are eligible for, and will help you access the services you need.

A preferred rate or free access to Kin On's health workshops and lectures.

Peace of mind: if you have enrolled either an elderly or disabled relative or yourself, you can begin to feel confident that Kin On will assist you in your safety and health needs that may arise in the future.
