Established
as a skilled nursing facility, Kin On Health Care Center has
continued to provide the community with high quality long-term
care over the years. Kin On recognizes the importance to
proactively evolve its services with the changing needs of the
community. To meet the transitional needs of individuals
discharged from the hospital, Kin On has implemented a
short-term rehabilitation program called Transitional
(Short-Term) Care Services.
Our Commitment
Our goal is
to provide short-term rehabilitative and special care services
to individuals discharged from the hospital so they can
successfully transition back to the community or other care
settings. The length of stay is usually a few weeks.
Our Philosophy of Care
Kin On
strives to provide a respectful, culturally sensitive and
resident-centered environment to facilitate the recuperation of
the resident. Services and plan of care are coordinated and
provided by a team of professionals to meet the unique
individual needs of each resident.

While the
purpose of this program is to provide short-term rehabilitative
care, a Social Service Staff will work closely with the client
and family members on discharge planning. Staff will also be
available to assist in accessing additional in-home services as
well as other transitional facilities based on the preferences
and needs of the client.
